Here is some important information to help you prepare for your visit with our women’s health physician Dr. Melissa Juliano, and/or our family nurse practitioners Becky Jensen and Kimberly Scott.
What to Bring to Your Appointment
Please bring the following to your appointment:
- Photo ID
- Insurance card (s)
- Co-payment, if needed
- Current list of all medications, including vitamins and supplements, or actual prescription bottles
- Appointment Cancellation Policy: We make every effort to schedule an appointment at a time that is convenient for you. As a courtesy to others who may require our services, please call our women's health primary care office if you need to cancel the appointment.
- No-Show Policy: Patients will be dismissed from our practice if there are three no-show visits (appointments not cancelled) in a rolling 12-month period of time.
- Late Policy: A patient’s appointment will be re-scheduled if the patient is more than 5 minutes late.
Prescription Refill Policy
First, please check with your pharmacy to see if you have additional refills remaining on your prescriptions. If you believe your prescription will run out before your next office visit, you may call our office to discuss your need for a refill. If you are due for a follow-up appointment before a refill, we will schedule your next appointment. Prescriptions will not be refilled after office hours or on weekends.
After-Hours Medical Care
If our women's health primary care office is closed and you need medical care, call our office main phone number at (419) 526.8877 to see if there is a physician on call. If not and if your medical need is urgent, please go to urgent care or the hospital emergency room.
